This Episode of Summit Cares Birdman talks to Dawn Marino RN and Julie Jackson RN about the Cardiac Pulmonary Rehabilitation and how it helps patients to recover and live better lives.

Help people who have a history of or are at risk for heart disease (cardiovascular disease) or are living with lung disease (pulmonary disease). People with heart disease can improve their health and reduce their risk of future cardiovascular disease, medical complications, and future hospitalizations. People with lung disease can learn to manage their disease well and reduce the risk of hospitalization.  The programs offered are designed to support a patient’s physical, emotional and social needs and include support for their families.

Summit Healthcare CardioPulmonary Rehabilitation programs serve Northeastern Arizona and use the guidelines of the American Association of Cardiovascular and Pulmonary Rehabilitation. These programs include a specialized plan of care developed by our team of experienced, advanced life support certified, registered nurses and licensed respiratory therapists.

In the CardioPulmonary Rehabilitation Services department, cardiologists and pulmonologists work with a multidisciplinary team to manage your care. The team may include:

  • Registered Nurses
  • Respiratory Therapists
  • Nurse Educator
  • Pharmacist
  • Registered Dieticians/Diabetic Educators
  • Occupational Therapist
  • Social Worker
  • Medical Technician

Programs offered by Summit Healthcare CardioPulmonary Rehabilitation:

INPATIENT PROGRAM INTRODUCTION

Hospitalized patients receive a personal visit from one of our staff to explain the rehabilitation programs, their benefits, and how to get involved. Medicare and most other insurances cover the program for specific conditions.

PHASE II PROGRAMS

On your first visit a registered nurse or licensed respiratory therapist does an evaluation. Patients receive a six (6) minute walking or seated exercise test to assess fitness.  Together the patient and staff make a personalized plan of care that includes exercise and learning. This plan is updated throughout the program.  The program lasts up to 36 sessions of exercise and education.  Patients get a wide variety of classes about how to manage their disease including: managing stress; proper medication use; how to shop, cook, and eat healthy; how to relax for better heart and lung function; how to manage heart and lung emergencies; how to manage weight loss; understanding asthma, emphysema, and COPD; and how to do daily activity without getting too worn out.

CARDIAC PROGRAM

Designed for people who have suffered a heart attack, coronary bypass (CABG), heart valve replacement, coronary stent, heart failure, or have stable angina. The program requires a healthcare provider’s referral.  Surgical notes, an EKG, and and echocardiogram (if available) should be sent with the order.  Monitoring during exercise sessions includes continuous telemetry (heart monitoring), periodic oximetry (oxygen measurements), and blood pressure checks.  At the end of 36 sessions, patients will have a better understanding of heart problems and how to keep themselves in the best possible health.

PULMONARY PROGRAM

Is designed for people suffering with lung problems from COPD (chronic obstructive pulmonary disease) including: bronchiectasis, chronic persistent asthma, pulmonary fibrosis, and emphysema.  The program requires a health care provider’s referral, and the results of a Pulmonary Function Test (PFT) should be sent with the order.  Each session of monitored exercise includes periodic oximetry (oxygen measurements), and blood pressure checks.  Sessions also include staff instruction on use and cleaning of home equipment, oxygen use, and proper use of an inhaler.  Patients’ are taught “pursed lip breathing” to help them feel less winded.  At the end of 36 sessions, clients will have a better understanding of lung problems and how to keep themselves in the best possible health.

COMMUNITY HEALTH AND FITNESS MAINTENANCE PROGRAM

Is for patients who have completed Phase II and for members of the community who want to exercise in a group setting with nurses available.  This maintenance program is for independent exercise.  Blood pressure, pulse, and oxygen checks are available on request.  This is usually not covered by insurance (clients are self pay).

The CAPD (Cardiac and Pulmonary Discussion) group meets weekly to talk about ways of dealing with chronic heart and lung disease.  This casual meeting lets patients meet others with problems like theirs.  Spouse or one support person is welcome. It creates a support network for patients and family members. MEETINGS ARE FREE OF CHARGE.

In this Episode of Find Your ChangePoint, Birdman meets Tara Lopez, who has a very interesting perspective when it comes to helping recovery clients. Kristi Iannucci also chats about ChangePoints unique ability to help those who are looking to better themselves and move forward.

 

Tara Lopez has worked with ChangePoint Integrated Health for over 7 years in a variety roles, and is currently working as a Recovery Support Specialist where she works with supporting adults in their recovery. She leads several groups at ChangePoint including the gardening group where adults are able to come and grow fresh produce to take home for nutritional eating. Tara also leads the efforts with ChangePoint’s vision center where individuals are able to come to relax, socialize, have access to a computer and get any general support they may need. Tara has years of experience working with children, adults and even the developmentally disabled. She is a valuable team member at ChangePoint.
